Immunizations represent a collection of related vaccines that can be tracked and reported. You can specify a reporting policy for each grade level to aid in reporting children missing the required immunizations.
Vaccines are the shots or doses that constitute the components of an immunization.
Select Students from the Jump to Module pull-down list in the upper right corner.
Select Immunizations from the Health menu, and the Immunizations window opens.
Click Add Immunization, and the New Immunization window opens.
Enter the Name, and click Create. The immunization is saved, and the Edit window opens displaying the Vaccines and Policies tabs.
Click on the Vaccines tab, and that window opens.
Click Add Vaccine, and the Vaccine editor opens.
Complete the fields as described.
Code: Enter an abbreviation for the vaccine — for example, DTaP, Td, and Tdap for the diptheria/tetanus/pertussis immunization.
Description: Enter the name of the vaccine — for example, Diphtheria, Tetanus, Pertussis for the DTaP.
Doses Tracked?: Enter the number of doses that will be tracked in the student's immunization record.
Click Save.
Click on the Policies tab, and that window opens.
Click Add Policy, and the Policies editor opens.
Complete the fields as described. See the Diphtheria/Tetanus/Pertussis example below to further explain the fields.
Grade Level: Select the grade level for this policy.
Entry Requirement: Enter the number of doses required prior to attendance.
Provisional
Threshold: This number may be the same as, or different
from, the entry requirement. It will be different when a lower
number of doses than the entry requirement can fulfill the requirement.
For example, if the entry requirement is 5 doses, but 4 doses would
meet the requirement if the 4th dose was received after a certain
birthday, then you'd enter 4 as the provisional threshold.
Age Threshold: If the provisional threshold is lower than the entry requirement, enter the age on or after which the "provisional" dose must be received. For example, if the entry requirement is 5 doses, but 4 doses would meet the requirement if the 4th dose was received after the 4th birthday, then you'd enter 4 as the age threshold.
Age Adjustment: This field designates the number of doses fewer than the entry requirement when there is a provisional threshold.
In the KG - 1st grade DTP example below, the entry requirement is 5 doses, but 4 doses would meet the requirement if the 4th dose was received after the 4th birthday, so you'll enter -1 as the age adjustment: 5 - 1 = 4.
Since 2nd - 6th graders are typically 7 years or older, 3 doses meet the requirement if 1 dose was received on or after the 4th birthday. So you'll enter -2 as the age adjustment: 5 - 2 = 3.
Booster Requirement: If required for this grade level, enter the number of years since the last vaccination at which a booster shot is required.
Click Save.
Requirements from the Texas Department of State Health Services:
For K - 6th grade: 5 doses of diphtheria-tetanus-pertussis vaccine; 1 dose must have been received on or after the 4th birthday. However, 4 doses meet the requirement if the 4th dose was received on or after the 4th birthday. For students aged 7 years and older, 3 doses meet the requirement if 1 dose was received on or after the 4th birthday.
For 7th grade: 3 dose primary series and 1 Tdap/Td booster within last 5 years.
For 8th - 12th grade: 3 dose primary series and 1 Tdap/Td booster within last 10 years.
